The menisci play a crucial role in load distribution, joint stability, and shock absorption. Damage to these structures may lead to pain, locking, swelling, and restricted range of motion. Accurate clinical diagnosis through physical examination is essential before advanced imaging is considered. Among the most commonly used special tests for meniscal assessment are the McMurray test, the Apley grinding test, and the squat test.<\/p>\n<h3 style=\"text-align: left;\" data-start=\"3674\" data-end=\"3691\">McMurray Test<\/h3>\n<p style=\"text-align: left;\" data-start=\"3693\" data-end=\"4657\">The McMurray test is one of the most widely used and well-known clinical tests for detecting meniscal tears. The patient lies in a supine position with the knee fully flexed. The examiner palpates the posterior joint line with one hand while holding the foot with the other.<br data-start=\"3967\" data-end=\"3970\" \/>To assess the medial meniscus, the tibia is placed in maximal external rotation, and the knee is slowly extended. A palpable or audible click occurring as the femoral condyle passes over a torn meniscus suggests a positive test.<br data-start=\"4198\" data-end=\"4201\" \/>For the lateral meniscus, the tibia is placed in maximal internal rotation, and the maneuver is repeated. The angle at which the click occurs is clinically relevant: a click between full flexion and approximately 90 degrees often indicates a posterior horn tear, while clicks occurring later in extension may suggest involvement of the meniscal body or anterior horn. It is important to note that a negative McMurray test does not rule out a meniscal tear.<\/p>\n<h3 style=\"text-align: left;\" data-start=\"4659\" data-end=\"4682\">Apley Grinding Test<\/h3>\n<p style=\"text-align: left;\" data-start=\"4684\" data-end=\"5347\">The Apley grinding test is performed with the patient in a prone position and the knee flexed to 90 degrees. The examiner stabilizes the patient\u2019s thigh against the examination table. First, the lower leg is distracted upward and rotated internally and externally. Pain during this distraction phase is more suggestive of ligamentous injury.<br data-start=\"5025\" data-end=\"5028\" \/>Next, a downward compressive force is applied through the tibia while rotational movements are repeated. Pain or abnormal sounds during compression indicate a possible meniscal lesion. The comparison between pain during distraction versus compression is key in differentiating meniscal pathology from ligament injuries.<\/p>\n<h3 style=\"text-align: left;\" data-start=\"5349\" data-end=\"5363\">Squat Test<\/h3>\n<p style=\"text-align: left;\" data-start=\"5365\" data-end=\"5873\">The squat test involves repeated squatting movements performed by the patient while the tibias are placed in internal and external rotation. Pain elicited during internal tibial rotation is commonly associated with lateral meniscal injury, whereas pain during external rotation is more suggestive of medial meniscal damage. However, one of the most important diagnostic findings during this test is joint line tenderness, which often provides more precise localization of the meniscal lesion than pain alone.<\/p>\n<h3 style=\"text-align: left;\" data-start=\"5875\" data-end=\"5889\">Conclusion<\/h3>\n<p style=\"text-align: left;\" data-start=\"5891\" data-end=\"6280\">McMurray, Apley, and squat tests are valuable components of the physical examination for suspected meniscal injuries. Nevertheless, none of these tests is definitive when used in isolation. Their findings must be interpreted alongside patient history, comprehensive clinical examination, and, when necessary, imaging studies such as MRI to ensure accurate diagnosis and optimal management.<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Meniscal injuries of the knee are among the most common orthopedic conditions, particularly in athletes and physically active individuals.
